We identified three paradoxes about the exclusion of vulnerable populations in low-and-middle income countries (LMICs) from research.
(1) Regulatory and governance issues lead to top-down exclusion;
(2) loss of trust due to historic and current exploitation in research leads to bottom-up exclusion, and
(3) the lack of low-risk research methods leads to process-related exclusions.
